Abstract

Treatment selection for very elderly patients with endometrial cancer (EC) is frequently constrained by multimorbidity and frailty, increasing the risk of undertreatment and compromised outcomes. With the incorporation of molecular profiling into clinical practice, a subset of ECs has been recognized to exhibit homologous recombination deficiency (HRD), providing a rationale for the use of poly(ADP-ribose) polymerase (PARP) inhibitors. However, evidence for PARP inhibitor maintenance in EC remains limited, particularly in nonagenarians.
